The Activity and Billing Statement is a detailed record of your plan payments and activity history, designed to help you get reimbursed for your wellness expenses.
For security purposes, this statement is available exclusively via the Wellhub app for Primary Account Holders who are active employees of a company that offers Wellhub.
📌 If you’re a Family Member or kept your plan after leaving a company, check your usage details directly in the app under Check-in History and Billing History.
How to request your statement
- In the Wellhub app, go to your Profile and tap the Settings gear icon.
- Select Account, and then Activity and Billing Statement.
- Choose the time period you need for your statement (up to a maximum of 12 months) and tap Submit request.
It can take up to 24 hours for your statement to be prepared. You'll receive an email notification as soon as it's ready.
💡 You can track the status of your request or download the file when it's ready from the Activity and Billing Statement section in your account. The download link will expire after 7 days.
The Activity and Billing Statement is a single PDF that contains two parts:
- Billing History: A record of all payments you have personally made for your subscription.
- Activity History: A record of all your validated check-ins for the selected time period.
